The answers you give on this application will help us to find the best possible match between you and the Goldens available through RAGofAZ. Please read our educational materials before filling out the adoption application. If you have not seen the materials, you may find them by clicking on "About RAG of AZ"
Please fill out the application completely. Please include area codes with all phone numbers, and please include your email address if at all possible. One of our representatives will be contacting you shortly after we receive your application.
An adoption fee to RAGofAZ is required at the time of adoption. This fee is used to cover aggregate costs of caring for the dogs. The fee amount is as follows:

• All dogs up to age 4, $400
• Dogs 4 - 8, $300
• 8 and over, $250
• Pairs -- second dog is half price:
  • both dogs younger than 4, $600
  • one dog younger than 4 and one between 4 and 8, $550
  • one dog younger than 4 and one over 8, $525
  • both dogs between 4 and 8, $450
  • one dog between 4 and 8 and one over 8, $425
  • both dogs over 8, $375

The fee may be adjusted for a very senior or special needs Golden Retriever. Your completed adoption application may be sent to us via the PO Box listed on the application, fax or email. Please complete all questions. If a particular question does not apply to you, please write N/A in the response area. Attach an additional page if extra space is required. Please print neatly or type all responses.
